What is a Federation?

Primary care federations play a crucial role in addressing the healthcare requirements of local communities. They serve as an expansion of primary healthcare services, uniting health, social care, and voluntary organisations, all within the proximity of individuals’ homes. Moreover, these federations form an essential component of the NHS Confederation’s enhanced primary care network.

This network amalgamates GP federations and primary care networks into a robust and unified platform for the healthcare sector.

OUR BACKGROUND

Lambeth GP Federation are  a group of GP Practices who work together to provide better local services for patients at scale. 

The Lambeth GP Federation consists of 41 GP Practices across Waterloo, Vauxhall, Kennington, Clapham, Brixton, Stockwell, Streatham Hill,  Streatham Common, West Norwood, Tulse Hill, Herne Hill, and Oval.

WHO WE ARE

Lambeth Healthcare, situated in Southeast London, serves as a GP Provider Organisation committed to collaborating closely with GP Practices in Lambeth. Our primary aim is to deliver exceptional and empathetic patient care on a substantial level.

The Lambeth GP Federation brings together the 41 GP Practices spread throughout Lambeth. Our leadership is composed of local clinicians and practice managers who form our board of directors, and we receive support from a dedicated operational core team.

WHAT WE DO

Our collaborative approach empowers federation boards to adopt a strategic perspective on how primary care can function effectively across various levels, while also working closely with system partners.  

Our partners benefit by having the opportunity to collaborate with a single organisation representing all practices and/or PCNs within a specific area.  This includes activities such as A&E triage in conjunction with hospital partners, accommodating community and social care staff within vaccination delivery centres, or leading multi-disciplinary teams (MDTs) at both the neighbourhood and intermediate care levels.  Crucially, this approach allows for consistent service delivery across the entire locality. 

Importantly, federations provide these services while utilising the same GP clinical records as their member practices, enabling them to offer care seamlessly as an extension of primary care, deeply intertwined with the practices and PCNs they represent—an original and comprehensive integration of clinical care.

WORLD CLASS HEALTHCARE

During the second wave of Challenge Fund allocation, Lambeth GP Federations received a £3.5 million grant, supplemented by an additional £1.5 million commitment from Lambeth CCG. This funding facilitated the launch of a patient access program, introducing primary care services seven days a week to the Lambeth community through the GP Access Hubs, as described on this website.

OUR VISION

Our vision is built on 4 main principles:

  • Attract investment into primary care to help sustain our member practices.
  • Support practices to work with other providers to provide better-integrated care for patients.
  • Make fair and transparent decisions about the allocation of resources to practices, providing services that represent value for money.
  • Build healthy relationships fairly and transparently to work together at scale and provide the best possible out-of-hospital care.
